receiving your request, we ask that you not use this form to inquire
about patients. If you have a question about a patient, or wish to
discuss a patient's care - especially if you feel the matter is urgent - please check our frequently called numbers directory to find the phone number for appropriate nursing unit or department and call for assistance.
During business hours, you are welcome to call Service Excellence Specialist at 704-873-5661, extension 3538. After hours, you can call the switchboard at 704-873-5661 and ask for the Nursing Shift Supervisor.
If you need a copy of your medical records
Please be advised, for reasons of privacy and identity protection, we cannot process requests for medical records received via this form or by email. For these same reasons, we cannot fax or email your records to you. Please call 704-878-4634 to make arrangements to obtain the documents you need. Thank you.
